Fairway Chalet ALF

Fairway Chalet is an assisted living facility dedicated to create a family atmosphere in a home-like setting that provides for the health, safety, and welfare of its residents. Not only does it have a standard license but it also has a specialty license, the limited nursing services license, issued by the Agency for Health Care Administration of the State of Florida (License #: AL7276). Residents have better chances of aging in place here. Works with home health and hospice agencies. It has the following: Spacious bedrooms with furnishings, towels, and linens available; Registered Dietician-prepared monthly menu of three well-balanced meals a day with available snacks; Basic laundry services available; Social and leisure activities such as birthday and holiday celebrations, games like bingo, and sing-along; House doctor available for routine visit and on as needed basis; Personal assistance with Activities of Daily Living (ADL) like grooming, transferring, ambulation, eating, and toileting, and medication management that is tailored to individual needs; Beautician services available; Assistance with doctor and other appointments and transportation set-up available. Residents may contract with outside agencies to bring in services (assistance available). It is a Medicaid Long Term Care Diversion Program participant. The Administrator/Owner is a Registered Nurse with a Bachelors degree and over 25 years experience with elder care. It has CPR-trained staff present 24 hours a day.
